Do I need an appointment?
Our offices work by appointment only to ensure that a certified practitioner will be available to assist you. Please call the appropriate regional center in Charlotte, Concord, Statesville, Monroe, Gastonia, Pineville, or Salisbury to set up an appointment for an evaluation. Our staff will schedule you for the earliest available appointment date.

Do I need a prescription?
In almost all cases, new patients should have a prescription before they are seen and should bring that prescription to their appointment. Due to insurance requirements, we are required to have a detailed prescription for every adjustment/repair/device that is completed. Much like a pharmacy, we can only issue prostheses/orthoses to patients who are under the care of a physician, and only in response to that physician’s orders.

How much does the appointment cost?
There is no charge for appointments at any of our clinic locations. Charges are incurred only upon receipt of a prosthetic/orthotic device or product, or in the event that a repair or adjustment is needed to a prosthesis/orthosis that is no longer covered under warranty.

Will my insurance cover my prosthesis/orthosis?
Our services are covered by Medicare, Medicaid, and most major insurance plans. However, because there are special rules regarding coverage, please discuss these matters with a patient representative during your initial visit. As a courtesy, Faith Prosthetic-Orthotic Services will file your insurance claims for you. With prior approval from the insurance carrier, we will generally accept your claim assignment. Please remember that your insurance coverage is an agreement between you and your insurance carrier. Our staff will attempt to verify your benefit information and provide you with an estimate for services. While we do our best to interpret your insurance guidelines for you, we are bound by the rules regarding your coverage.

What do I need to bring to my first appointment?
You should bring your prescription and a current copy of your insurance card(s). All new patients will be required to fill out a new patient packet that can be downloaded by clicking here. Having the forms completed in advance will save you time on the day of your evaluation. Returning patients will need to update these forms each year.

How long will it take to get my prosthesis/orthosis?
In most cases, a new patient visit is the result of the requirement for a physician-prescribed, custom-fabricated device. Generally, more than one visit will be required before the final delivery is made, with the actual number of visits determined by the patient’s particular needs. Usually, subsequent visits are scheduled at one-to two-week intervals.

Why can’t I just pick up my prosthesis/orthosis?
An orthosis or prosthesis is a custom product. We need to determine that the fit and function is appropriate and explain proper care and use for the appliance. In many cases, insurance companies require that a certified prosthetist or orthotist fit the device and document accordingly.

What if I need an adjustment to my prosthesis/orthosis?
Please call the appropriate regional center to schedule an appointment. During the first 90 days following delivery of the prosthesis/orthosis, visits are at no charge. After this period, you may be charged for adjustments, modifications, and/or repairs.

Are your components covered under a warranty?
Most materials used in the fabrication of prostheses/orthoses are warranted by the manufacturers. Fabrication workmanship is warranted against defects for a period of one year.

Am I able to set up a payment plan?
Full payment is expected at the time of completed service. If you are unable to pay in full at the time of service, it may be possible to work out a special payment plan through a patient representative.

Our practitioners and facilities are certified by the American Board for Certification (ABC).

What is HIPAA and how does it affect me?
HIPAA stands for Health Insurance Portability and Accountability Act of 1996. The goal is to improve efficiency in healthcare delivery by standardizing electronic data interchange and protecting the confidentiality and security of healthcare data. Your information and progress reports will not be released to other parties without prior written consent. You may obtain copies of your records upon request and completion of the appropriate paperwork.
