amputee mobility predictor scores?

I have been researching the possiblity of using (and/or finding a Dr. to use) the Amputee Mobility Predictor. However, so far I have been unable to find what final score ranges put a person in what specific functional category (0 through 4)?  Any expertise would be appreciated. Todd Norton, LP, CP
