Physician evaluation for prosthesis

Hello All,   We have a prosthetic patient that is going to her physician for prosthetic evaluation.  I would like to get it done right the first time.  Is there an evaluation form out there that ensures the physician hits all the bases?  Any help would be appreciated.  Thank you for taking time.  Kevin Kevin C.
