Toe ( 4th ) flexion splint/ brace

Hi All I have a patient who recently had an operation on her 4th toe and as a result is starting to deviate into an extension contracture. At present I am using both a custom silicone orthosis and the Darco TAS. However the contracture is too great for both silicone and the TAS. She is an active middle aged woman. PS The TAS works but the strap around the toe is too narrow and the toe starts to swell distal to the strap. Any ideas on an alternative treatment option. Kind regards Gary Seaman MSc.
