Question about Ossur Locking Liner

We have a patient with an Ossur locking liner that disengages the pin with out pushing the stem. Stem and locking mechanism has both been replaced. The liner still unlocks it's self even when the patient is sitting. Has anyone had the same problem. Please advise.
