Amputee Patient Checklist

Before working with any prosthetist,
ask him or her the following questions:
Yes No
1. Are you a Certified Prosthetist (CP)?
2. Is your credentialing organization the American Board for Certification in Orthotics and Prosthetics (ABC)?
3. Are you bound to the Canons of Ethical Conduct- standards of ethics, character and fitness which hold you accountable for your responsibilities to me, my physician and your profession?
4. May I see your ABC certificate before we proceed?
5. Have you passed a series of rigorous examinations designed to evaluate your educational and professional competency?
If your prosthetist answers "no" to any of the above questions, the United Amputee Services Association (UASA) and the American Board for Certification in Orthotics and Prosthetics (ABC) want you to know that you deserve - and can receive - high quality care.
