foot shell replacements

How are you all getting foot shell replacements covered? I've billed for labor & materials and get anywhere from $30-$60 total (combo of both codes) regardless of the amount billed. Have had success billing the foot Lcode, a reduced fee and the repair modifier. This pays, but I fear that when the patient needs to replace the WHOLE foot, it will be denied because I've already bill the foot Lcode for the cover. Suggestions welcome. Can't continue to service these feet, replace bumpers, bushings & covers and get $30. Pretty consistent problem across all insurances, so wondering how others are getting these repairs covered.
