US Politics - mid-year L-code file revision

Dear Colleagues, I've just reformatted the July 2016 revised CMS L code Excel file. I do this formatting for my own use, but I make it available to colleagues who might find it useful. All the information is from CMS. I've merely re-organized it. I offer it for free. If you find errors, keep in mind that I charge nothing and I accept no liability - but please alert me.
