Extended warranty for microprocessor knees

I have a couple questions and would appreciate some feedback;

1. How frequently are the 2 additional years of coverage for microprocessor knees purchased?

2. How is the cost absorbed OR how is it billed/reimbursed or charged to the client successfully?
