Patient Instruction sheets

We are seeking sample Patient Instruction Sheets: we are in process of editing existing & creating new sheets, for variety of both Pre-fab & Custom devices including spine (TLSO, LSO, CO, etc), Knee, AFO's, Footwear. These would accompany or replace manufacturer sheets. Any samples or resource references would be welcome.
