K-Level Assessment Form for MD-s

*We have been successful in having physicians assess patients' functional
level using a form, which we designed in a questionnaire format. We also
had a few instances, when Medicare requested additional documentation and
was satisfied when this form was submitted. Anyway, this is just a simple
tool that may help you, together with all other paperwork, document
K-levels.*
