Would like to get an idea of what facility warranty policies are out
there.

Specifically would like to know how folks warranty...

l. Custom orthotic devices. Are straps and padding handled
differently from the superstructure of the device? How long is the
warranty for and what is covered.

Copies of or examples of warranty forms for any prosthetic and
orthotic devices would be appreciated.

2. Custom prosthetic devices.

Do warranties differ from medicare to private insurance. If the
prosthesis is warrantied for x amount of time, how much time before
charges are made for perishable items like leather
belts, liners, suspension sleeves, spectra socks etc.

Do you have a 'minimum' labor charge or is it always l5 minute
increments?

3. Does medicare have specific guidelines for how long a brace or
limb is considered under warranty and at what point can charges be
made for labor, adjustment,
repairs and supplies?
